What is the Death and Funeral Benefits Application (OCF-4) ?

The Financial Services Regulatory Authority of Ontario (FSRA) has a series of Auto Insurance Claims Forms (OCF Forms) that need to be filled out if you are seeking compensation or benefits for your injuries. The OCF-4 form also known as the Death and Funeral Benefits Application is a form that needs to be completed in full and submitted to your auto insurance provider on behalf of the spouse and/or dependant(s) of the deceased and any other person entitled to claim for benefits. If more than one individual is applying for benefits, they can apply together or separately.

Additionally, you will also be required to complete the Application for Accident Benefits form as well. The Death and Funeral Payments Benefit is divided into two payment types. Death benefits are paid as a lump sum depending on the familial relationship and insurance policy. Funeral benefits on the other hand are provided as a reimbursement for funeral expenses and generally anywhere from $6,000 to $8,000.

What information is required on the OCF-4?

Firstly, information on the deceased is required including a copy of the official Death Certificate. The form also asks you to list all the dependant(s) and spouse of the deceased and how they are related. Original receipts of expenses relating to the funeral are also required and must be itemized on the form. If any receipts are missing you will need to outline details of what is missing and why they haven’t been submitted with the form.
