Form 1: Assessment of Attendant Care Needs

The Financial Services Regulatory Authority of Ontario (FSRA) has a series of Auto Insurance Claims Forms (OCF Forms) that must be completed to apply for accident benefits after a motor vehicle accident.

In addition to the standard OCF Forms required under Ontario’s Statutory Accident Benefits Schedule (SABS), a Form 1: Assessment of Attendant Care Needs must be completed if you are claiming attendant care benefits from your insurance provider.

This form is used to determine whether you need assistance with the tasks of daily living as a result of your accident injuries. It will also determine how much financial support you’re eligible to receive due to the effect your injuries have had on your life. It must be completed by an occupational therapist or registered nurse and submitted to your insurance company for approval before coverage is provided.

What Information is Required on Form 1?

Form 1 provides a breakdown of the claimant’s care needs and calculates the monthly amount they need to hire attendant care services. You’ll need to submit information including:

  • Your personal and insurance details
  • A detailed assessment of your physical, cognitive, and emotional functioning
  • The type and frequency of assistance you require for daily tasks, like bathing, getting dressed, using the washroom, eating, and moving
  • Calculated monthly costs based on hourly rates of caregivers and levels of care
  • Your healthcare provider’s reasoning for each care service you require

Your auto insurer will use this information to determine whether you’re eligible for attendant care benefits and how much they’re willing to cover.
