OCF-18: Treatment and Assessment Plan


What is the Treatment and Assessment Plan Form (OCF-18) ?

The Financial Services Regulatory Authority of Ontario (FSRA) has a series of Auto Insurance Claims Forms (OCF Forms) that need to be filled out if you are seeking compensation or benefits for your injuries. The OCF-18 form also known as the Treatment and Assessment Plan Form is a form that needs to be completed in full and submitted to your auto insurance provider by your attending health care professionals. The Treatment and Assessment Plan form also requires your healthcare professionals to include all goods and services contemplated. This form can be filled out by health practitioners such as chiropractors, dentists, nurse practitioners, occupational therapists, optometrists, physicians, physiotherapists and speech language pathologists.

What information is required on the OCF-18?

The form requires numerous parts to be filled out. For example, in addition to providing information on the applicant and insurer information by yourself – your healthcare practitioner is also required to fill out a treatment plan that includes all goods and services contemplated by the regulated health professional. For accidents that occured on or after September 1, 2010, the form asks your healthcare practitioner to confirm if the impairment or injury is also referred to in the Minor Injury Guideline and applicable to the subject accident. If the healthcare practitioner answers YES to this section, then they are also asked to confirm if the treatment under the Minor Injury Guideline has already been provided and if additional treatment goods are required within the $3,500 limit. They will also need to disclose if the applicant has any pre-existing medical conditions that were documented by them or another health practitioner before the accident that will prevent the application from achieving maximal recovery from the minor injury if the applicant is subjected to the $3,500 limit or is limited to the goods and services authorized under the Minor Injury Guideline.
