OCF-21: Auto Insurance Standard Invoice

The Financial Services Regulatory Authority of Ontario (FSRA) has a series of Auto Insurance Claims Forms (OCF Forms) that must be completed to apply for accident benefits after a motor vehicle accident.

The OCF-21 form, also known as the Auto Insurance Standard Invoice, is used by healthcare providers to bill your insurance company directly for approved goods and services related to your accident and recovery. It needs to be submitted after your treatment plan (in an OCF-18 form) has been approved by your insurer, or when charging for treatment you’re receiving under the Minor Injury Guideline (MIG).

What Information is Required on the OCF-21?

The OCF-21 will need to be filled out by your healthcare practitioner and includes information such as:

  • Your name and insurance claim details
  • Dates of services provided
  • Your healthcare provider’s information (including their licensing details)
  • A breakdown of fees charged for specific services or goods (MRIs, surgeries, x-rays, assistive living devices, etc.)
  • Confirmation that the invoiced treatment was approved or complies with MIG (if applicable)
  • Signatures from yourself and your healthcare provider

The completed OCF-21 form will be used by your insurer to process payments directly to your provider.
