OCF-2: Employer’s Confirmation Form

The Financial Services Regulatory Authority of Ontario (FSRA) has a series of Auto Insurance Claims Forms (OCF Forms) that need to be filled out if you are seeking compensation for your injuries. The OCF-2 form, also known as the Employer’s Confirmation form, is a form that must be completed by you and your employer(s). Your employer is responsible for submitting the OCF-2 directly to your insurance provider.

The OCF-2 form must be submitted to obtain income replacement benefits when your accident and/or injuries prevent you from working. These help reimburse insured victims for a specified percentage of their lost wages.

What Information is Required on the OCF-2?
