OCF-6: Expenses Claim Form

The Financial Services Regulatory Authority of Ontario (FSRA) has a series of Auto Insurance Claims Forms (OCF Forms) that need to be filled out if you are seeking compensation or benefits for your injuries after a motor vehicle accident.
The OCF-6 form, also known as the Expenses Claim form, is a form you can submit to your auto insurance provider to claim coverage for expenses you’ve incurred as a result of your accident. These expenses must not have been submitted for coverage by your healthcare provider in another form.
The OCF-6 Expenses Claim form is not included in the “standard” Accident Benefits Application Package (OCF-1), which you’ll receive when you notify your insurance company of your accident. If you wish to claim expenses not covered in your other forms, you must actively request the OCF-6 form.

What Information is Required on the OCF-6?
In addition to your personal information (name, address, phone number), the OCF-6 Form requires a list of goods and services you wish to claim as expenses resulting from your accident. This list will need to include a description of each good and/or service, including the names of service providers. The form also requires you to attach bills and receipts from the goods and services you’re claiming. If receipts are not available, you must provide an explanation as to why.
Examples of goods and services you may be able to claim on the OCF-6 include:
- Clothing or personal items that have been lost, damaged, or destroyed in your accident
- Assistive devices required as a result of your accident (e.g., walkers, prosthetics, wheelchairs)
- Travel expenses for visitors who are required to help in your recovery.
- Lost educational expenses (e.g., non-refundable tuition, if you are unable to attend classes due to your injuries)
- Transportation costs incurred as a result of your injuries
- Housekeeping and home maintenance (optional)
- Caregiver and/or attendant care benefits (optional)
- And more
