OCF-13: Declaration of Post-Accident Income and Benefits

​The Financial Services Regulatory Authority of Ontario (FSRA) has a series of Auto Insurance Claims Forms (OCF Forms) that must be completed to apply for accident benefits after a motor vehicle accident.

The OCF-13, also known as the Declaration of Post-Accident Income and Benefits, is a form used to report any income you have received after your accident, as well as any other benefits you’ve received (like CPP, EI, or private disability insurance). This form helps insurers calculate the amount of accident benefits you’re eligible to receive, including income replacement benefits, considering what you have already received from other sources.

What Information is Required on the OCF-13?

This form must be filled out by you and may require documentation that supports your statements. The OCF-13 will ask about:

  • Your personal and insurance policy information
  • Income you received after the accident (e.g., salary, commissions, bonuses, tips)
  • Other benefits you’ve received since the accident
  • The amount, source, and frequency of those alternative payments

Providing inaccurate or incomplete information on the OCF-13 may delay or reduce the benefits you’re entitled to. It’s important to note that any information that is purposefully left out or misrepresented may be considered a criminal offence.
