OCF-24: Minor Injury Treatment Discharge Report

​The Financial Services Regulatory Authority of Ontario (FSRA) has a series of Auto Insurance Claims Forms (OCF Forms) that must be completed to apply for accident benefits after a motor vehicle accident.

The OCF-24 form, also known as the Minor Injury Guideline (MIG) Discharge Report, is completed by a licensed healthcare provider when your treatment under the MIG finishes. It’s used to report the outcome of your treatment, document progress you’ve made, and help assess whether you need additional care outside of the MIG framework.

What Information is Required on the OCF-24?

The OCF-24 will be filled out by the healthcare practitioner who recommended and provided your treatment to you. They’ll need to outline information such as:

  • Your personal and insurance information
  • The dates you received treatment
  • The date you were discharged from treatment
  • A summary of your diagnosis and the different types of treatment you received
  • Your current condition and how treatment has improved your function
  • Recommendations they have for follow-up care or additional treatment
  • Their name, registration number, and/or licensing information

Ultimately, the OCF-24 helps your insurer determine whether you need further treatment, and whether it should fall outside the MIG limits. If it does, you may be entitled to more benefits and compensation for the injuries you’ve sustained due to your motor vehicle accident.
