OCF-3: Disability Certificate

The Financial Services Regulatory Authority of Ontario (FSRA) has a series of Auto Insurance Claims Forms (OCF Forms) that need to be filled out if you are seeking compensation for your injuries after a motor vehicle accident.
The OCF-3 form, also known as the Disability Certificate, is a form that needs to be completed in full and submitted to your auto insurance provider if you were involved in an accident on or after November 1, 1996. It allows you to provide more information about the injuries or conditions you sustained in your motor vehicle accident. Claimants are required to fill out Parts 1 to 3 and then have their health practitioner fill out the rest of the form. The health provider must submit the completed form directly to the insurance company. Only authorized health practitioners are authorized to fill out this form, such as:
- Chiropractors
- Dentists
- Nurse practitioners
- Occupational therapists
- Optometrists
- Physicians
- Physiotherapists
- Psychologists
- Speech-language pathologists

What Information is Required on the OCF-3?
Parts 1 to 3 will require you to detail your personal information, insurance company information, as well as a description of the accident. It’s important that you don’t sign the form if you are unsure of any information or questions included.
The remainder of the form must be filled out by an authorized health practitioner. It will ask them to outline:
- Any injury and sequelae information
- Disability tests you’ve undergone and your symptoms
- Information from any previous examinations
- Investigations or consultations not previously reported
- If any further examinations are required
- Prior and concurrent conditions you may have
- Information on any pre-existing disability benefits
- Details about pre-existing conditions or injuries
- A list of medications you take
- Any other information related to your health
