OCF-5: Permission to Disclose Health Information

The Financial Services Regulatory Authority of Ontario (FSRA) has a series of Auto Insurance Claims Forms (OCF Forms) that need to be filled out if you are seeking compensation or benefits for your injuries after a motor vehicle accident.

The OCF-5 form, also known as the Permission to Disclose Health Information form, is required if your insurer asks you for medical records or health information from your healthcare providers or other relevant sources. This form gives the insurance company legal authorization to access your personal health information when assessing your claim for accident benefits.
