OCF-23: Treatment Confirmation Form

The Financial Services Regulatory Authority of Ontario (FSRA) has a series of Auto Insurance Claims Forms (OCF Forms) that must be completed to apply for accident benefits after a motor vehicle accident.

The OCF-23 form, also known as the Treatment Confirmation form, is used when a claimant has sustained a minor injury that falls under the Minor Injury Guideline (MIG). It must be completed by a licensed healthcare practitioner before your treatment can start, and is typically used in place of the OCF-18, which is more detailed and for more serious injuries. The OCF-23 may be used to have treatment for minor injuries such as whiplash, sprains, or strains approved.

What Information is Required on the OCF-23?

The OCF-23 must be completed and submitted by a regulated healthcare provider and will include:

  • Your personal and insurance claim information
  • A description of the accident and the nature of your minor injuries
  • Details of the treatment plan being proposed under the MIG
  • Estimated costs and length of treatment
  • The credentials and licensing information of your doctor
  • Signatures from yourself and your doctor

Before you can start any treatment under the MIG, your insurance company must be notified using the OCF-23 form.
